“My wisdom teeth don’t hurt, so they must be fine.”
Pain is a warning sign. But it’s not the only one. By the time a wisdom tooth starts hurting, damage may already be done to your other teeth, your gums, or your jawbone. At Oral Facial Surgery of Delaware, Dr. Campbell and Dr. Rafetto have removed thousands of wisdom teeth. And they’ve seen plenty of cases where patients felt zero pain, but their X-rays told a very different story.
How do you know if your wisdom teeth are trouble? Here are five warning signs that have nothing to do with pain.

Sign #1: Your Other Teeth Are Shifting
Have you noticed your front teeth getting crowded or overlapping? Maybe your retainer doesn’t fit the way it used to. That’s a red flag.
When a wisdom tooth tries to come in, but there’s no room, it pushes against the molars in front of it. That pressure can shift your entire bite over time. Orthodontists see this all the time. Patients who had perfectly straight teeth after braces, only to watch them crowd again in their early twenties.
If your smile is changing, your wisdom teeth might be the culprit. An X-ray can confirm it.
Sign #2: You Can See the Tooth, But It’s Not All the Way Through
A partially erupted wisdom tooth is a problem waiting to happen.
When a tooth breaks through the gum just a little, it creates a flap of tissue. That flap is a perfect hiding spot for food particles and bacteria. You can’t reach it with a toothbrush. You can’t floss under it. And that means infections, swelling, and bad breath are just a matter of time.
Dentists call this condition pericoronitis. It can flare up again and again. And each time, it gets harder to treat.
Sign #3: Your X-Ray Shows a Cyst or Tumor
This one is serious, and you’d never know it without imaging.
Sometimes, the sac of tissue around an impacted wisdom tooth fills with fluid and forms a cyst. That cyst can slowly hollow out your jawbone. It can damage the roots of nearby teeth. In rare cases, it can even form a tumor.
Cysts don’t hurt. You can’t see them. You can’t feel them. They only show up on a dental X-ray. That’s why we recommend a wisdom tooth evaluation in the late teens or early twenties, before problems have a chance to develop silently.
Sign #4: You Keep Getting Gum Infections or Bad Breath
Does your breath struggle to stay fresh no matter what you do? Do your gums feel tender or swollen in the back of your mouth?
Partially erupted wisdom teeth are notoriously hard to keep clean. Bacteria hide under the gum flap and along the backside of the tooth. This leads to chronic gum inflammation, persistent bad breath, and recurring infections.
If you’re constantly battling these issues in the back of your mouth, your wisdom teeth may be the root cause.
Sign #5: Your Dentist Has Been Watching Them for Years
If your dentist in Delaware has been “keeping an eye” on your wisdom teeth for more than a couple of years, it’s probably time for a specialist’s opinion.
General dentists are great at spotting potential problems. But oral surgeons are the ones who evaluate the position of the tooth, the health of the bone, and the proximity to nerves. We can tell you definitively whether the tooth should stay or go.
Waiting and watching is fine for a while. But eventually, you want answers.
